The problem is simply that the FXS port stopped working - in the form of no longer providing a dial tone. As I have said and as you can see from logs and settings both FXS ports are configured the same. Asterisk says it's working, just no tone. And the situation is that one port has tone and one doesn't. The one that doesn't work was working with the current configuration, and suddenly stopped having a dial tone on it.
My though is that something on the FXS board stopped working, ala no dialtone. Is it possible for a FXS board to fail in such a way?

i am not sure what happened with the FXS, but i never see such case happened. maybe, you can try to take it out and plug in to other slot on the mother board.

Changed it and same thing in another slot. Moved it back.
Ran fxstest on the 2 FXS ports I have. The stats report the same things, the regdump looks similar - but I don't know what it means, on the working one playing tones work, on the non-working one no tones are played (rather I can't hear anything). Don't know if this may help you, but here is the output:
